March 20, 2026The Effectiveness of Cognitive Behavioral Therapy in the Management of Functional Abdominal Pain Disorders in Children: A Systematic Review
Abstract
Background
Functional abdominal pain disorders (FAPDs) are among the most prevalent gastrointestinal conditions in pediatric populations, significantly impacting physical health, psychological well-being, and quality of life. Traditional management strategies, primarily pharmacological, have demonstrated limited effectiveness, leading to increased interest in psychological interventions such as cognitive behavioral therapy (CBT). Given the biopsychosocial nature of FAPDs, CBT has emerged as a promising treatment targeting both physiological and psychological mechanisms.
Objective
This systematic review aims to evaluate the effectiveness of cognitive behavioral therapy in reducing abdominal pain and improving quality of life and psychological outcomes in children and adolescents diagnosed with functional abdominal pain disorders.
Methods
A systematic review was conducted in accordance with PRISMA guidelines (Page et al., 2021). A comprehensive search of the PubMed database was performed for studies published between 2013 and 2023. Inclusion criteria focused on randomized controlled trials and systematic reviews involving pediatric populations and CBT interventions. A total of 50 records were identified, of which 10 studies met the inclusion criteria after screening and full-text assessment. Data extraction and quality assessment were conducted using standardized procedures, including the Cochrane Risk of Bias Tool (Higgins et al., 2011).
Results
The findings consistently demonstrate that CBT significantly reduces abdominal pain intensity and frequency in pediatric patients with FAPDs. Improvements were also observed in secondary outcomes, including quality of life, anxiety, depression, and functional capacity. Internet-based CBT interventions were found to be as effective as traditional face-to-face therapy, highlighting the potential for scalable treatment delivery. The overall effect size for pain reduction was moderate (SMD ≈ -0.54), as reported in meta-analytic evidence (Korterink et al., 2015).
Conclusion
Cognitive behavioral therapy is an effective and evidence-based intervention for the management of functional abdominal pain disorders in children. Its ability to address both psychological and physiological dimensions of the condition supports its integration into standard pediatric care. Future research should focus on standardizing treatment protocols and evaluating long-term outcomes.

1. Introduction
The functional abdominal pain disorders (FAPDs) according to the Rome IV criteria include irritable bowel syndrome (IBS), functional dyspepsia and functional abdominal pain not otherwise specified. Such disorders are typified by incessant or repetitive pain in the abdomen with no structural or biochemical defects (Hyams et al., 2016). FAPDs prevalence in children varies among different countries globally, and estimates identify a range of 13 to 20 percent as a leading health issue of concern (Korterink et al., 2015). FAPDs have a complicated and multifactorial pathophysiology, which includes gut -brain axis dysregulation, visceral hypersensitivity, altered motility, and psychosocial factors (Van Tilburg et al., 2015). Often, psychological comorbidities, such as anxiety and depression, are identified, and they play a significant part in the severity and persistence of their symptoms (Shelby et al., 2013). Conventional management interventions have strongly been based on pharmacological interventions; these interventions are not as effective and they are characterized by undesirable side effects (Kaminski et al., 2011). This has resulted in a paradigm shift of biopsychosocial models of care, shifting focus on psychological interventions including cognitive behavioral therapy (CBT). CBT is a goal-focused, systematic type of therapy which tries to alter the maladaptive thoughts, behaviors, and emotional reactions. CBT focuses on perceptions of pain, coping mechanisms, and illness anxiety in the case of FAPDs (Levy et al., 2017). The recent developments have also made it possible to provide CBT via digital channels, which makes it more accessible and scalable (Palermo et al., 2020). Despite the fact that several studies have conducted research on CBT when applied in pediatric FAPDs, the outcome of such studies is mixed in magnitude and methodological strength. Consequently, a logical review of existing research is justified in order to demystify its clinical efficacy.
2. Literature Review
2.1 Functional Abdominal Pain Disorders in Children: Pathophysiology and Clinical Burden
Functional abdominal pain disorders (FAPDs) are a collection of persistent or recurring gastrointestinal diseases resulting in persistent abdominal pain, lack of structural or biochemical pathophysiology. Ibsen et al. (2016) state that such disorders are irritable bowel syndrome (IBS), functional dyspepsia, abdominal migraine, and functional abdominal pain not otherwise specified, according to the Rome IV criteria. They belong to the most widespread gastrointestinal pediatric disorders, with the prevalence rates of 13-20 percent worldwide (Korterink et al., 2015). Although they are benign regarding mortality, FAPDs cause a significant burden to the children, families, and health systems. The pathophysiology of FAPDs is heterogeneous and multifactorial and includes biological, psychological and social interactions. The key concept applied within this framework is the gut–brain axis, which is a two-way communication system between the gastrointestinal tract and the central nervous system (Mayer, 2011).
This axis is suspected to have a role in dysregulation in visceral hypersensitivity, altered gut motility, and increased pain perception. The persistence of symptoms has been attributed to neurobiological processes such as dysregulated enteric nervous system signaling and central pain processing (Van Tilburg et al., 2015). Psychological factors are also very important in the development as well as the persistence of FAPDs. Children who have such conditions often have high rates of anxiety, depression, and somatization in comparison with the healthy ones (Shelby et al., 2013). These mental comorbidities are not the effects of chronic aches but part of the disorder as they affect the severity of symptoms and functional deficiency. As an example, anxiety can enhance the perception of pain by attentional stress on the sensations of the body, and maladaptive coping behaviors may maintain the cycle of symptoms (Eccleston et al., 2014). Moreover, the environmental factors that impact the clinical presentation of FAPDs include family processes, parental reactions to pain, and the school-related stressors, which significantly contribute to the presentation of the problems in addition to individual psychological factors. Illness behaviors such as parental reinforcement of such behavior have been found to raise disability and medical use among the affected children (Levy et al., 2010).
This underscores the need to have the bigger psychosocial picture in assessment and treatment. FAPDs do not only have a physical clinical burden. Children may have serious dysfunctions in daily living, such as lower school attendance, lower involvement in social activities, and lower quality of life (Varni et al., 2015). These effects may remain well into adulthood, and longitudinal cohort studies have shown that a significant percentage of individuals with such impairments would still report having chronic pain and psychological problems later in life (Walker et al., 2012). Healthcare wise, FAPDs have been linked to high use of medical services, such as visiting a physician frequently, testing, and hospitalization. Although there are still detailed studies, a definite organic diagnosis is not always provided to patients, which frustrates the families and the clinicians (Kaminski et al., 2011). This highlights the importance of sound management strategies that will deal with the underlying biopsychosocial processes but not emphasize on symptom relief only. The common treatment methods used in traditional treatment of FAPDs include pharmacological intervention, alterations in their diets and reassurance. Nevertheless, the effectiveness of pharmacological intervention in children is still low, and the issues of side effects further restrict it (Korterink et al., 2015).
Consequently, non-pharmacological interventions especially psychological therapies have increasingly been given prominence. Cognitive behavioral therapy (CBT) has become a promising treatment modal in this respect. CBT attacks major mechanisms that underlie FAPDs by focusing on maladaptive thoughts, behaviors, and emotional reactions. The combination of psychologic and physiologic approach to CBT is consistent with the biopsychosocial approach and is thus especially appropriate in managing functional disorders.
2.2 Cognitive Behavioral Therapy in Pediatric Functional Gastrointestinal Disorders
Cognitive behavioral therapy (CBT) is a psychological intervention, which is time-focused and structured and aimed at determining and altering maladaptive behavioral and cognitive patterns. CBT was originally used to treat depression and anxiety but has since been modified to treat a large number of medical illnesses including chronic pain disorders like FAPDs (Beck, 2011). The basis of its use in pediatric gastroenterology is the acknowledgement of centrality of psychological factors in the experience and maintenance of gastrointestinal symptoms. The theory elucidating the principles of CBT among FAPDs is closely interconnected with the biopsychosocial model, according which the biological, psychological, and social variables interact, thereby determining the health outcomes. In this context, CBT will focus on breaking the pattern of pain, distress, and dysfunctional coping by encouraging more adaptive behavioral and thinking reactions. CBT methods typically used to treat FAPDs are cognitive restructuring, relaxation training, exposure therapy, and coping and problem-solving skills training (Palermo et al., 2020).
There is an increasing amount of evidence that CBT is effective in alleviating pain and enhancing functional outcome in children with FAPDs. Randomized controlled trials have reliably shown that CBT results in the notable decrease of the intensity and frequency of pain in comparison to standard care or waitlist controls (Levy et al., 2017). Another advantage of the intervention is multidimensional since these effects are often accompanied by a better quality of life and psychological well-being. The restructuring of pain-related cognitions is one of the most important mechanisms by which CBT has its influence. Children with FAPDs are known to be catastrophic in their thinking patterns including overestimating the magnitude of pain or even foreseeing the bad things. CBT allows patients to recognize and dispute these distorted thoughts and substitute them with more realistic and sane views (Eccleston et al., 2014). This mental change is capable of decreasing anxiety and diminishing the emotional amplification of pain. CBT behavioral elements are vital. As slowing down the exposure helps children to resume their regular activities gradually, methods like the graded exposure provide an opportunity to minimize the avoidance behavior and enhance functional capacity. Relaxation methods, such as diaphragmatic breathing and progressive muscle relaxation, enable physiological arousal regulation and the minimization of the symptoms exacerbation of stress (Palermo et al., 2020).
The issue of family involvement is an important element of CBT among children. Parents are important in the development of children in regards to how they respond to pain and maladaptive parental behaviors like providing excessive reassurance or encouraging illness behaviors can continue the symptoms. CBT interventions based on a family focus endeavor to alter these relationships by teaching the parents and encouraging supportive and adaptive reactions (Levy et al., 2010). Internet-delivered CBT (iCBT) as a form of enhancing accessibility and scalability has received growing interest over the last few years. There are a number of benefits associated with digital interventions, such as low costs, increased convenience, and the capacity to target underserved populations. It has been demonstrated that iCBT may be as effective as offline therapy in terms of symptoms reduction and quality of life improvement (Lalouni et al., 2019). It is specifically applicable to the situation of healthcare systems with the lack of resources and rising demands on psychological services. Although the evidence base is increasing, numerous problems have been encountered during the application of CBT to FAPDs.
The first problem is that intervention protocols may be heterogeneous, that is, they may vary in their duration, intensity, and techniques used. This inconsistency complicates the direct comparison of outcomes of studies and standardization of treatment guidelines. Also, small sample sizes, absence of blinding, and the inconsistency of measuring outcomes are the methodological limitations that may compromise the quality of evidence (Higgins et al., 2011). Long-term sustainability of treatment effects is another factor of significance. Many studies have found an improvement that is significant, immediately after CBT, but not as many have conducted research after a period of one year. The longitudinal studies are required to ascertain whether the advantages of CBT are sustained and what factors contribute to the success in the long-term (Walker et al., 2012). Altogether, CBT is a justified and theoretically supported intervention in the case of pediatric FAPDs. It is a useful part of a complex care as it can tackle not only psychological, but also physiological problems of the disorder. Nevertheless, additional studies are required to streamline intervention schemes, enhance the quality of methodology, and increase the overall applicability of results.
3. Methods
3.1 Study Design
This was a literature-based systematic review (study), which was designed and reported in line with the Preferred Reporting Items of Systematic Reviews and Meta-Analyses (PRISMA) guidelines (Page et al., 2021). The PRISMA framework offers a systematic and clear way of finding, choosing, evaluating, and integrating evidence, thus, providing methodological rigor and reproducibility. The implementation of this framework was especially suitable considering the aim of the synthesis of findings of several randomized controlled trials (RCTs) to analyze the effectiveness of cognitive behavioral therapy (CBT) with respect to pediatric population with functional abdominal pain disorders (FAPDs). The systematic review approach was chosen because it would help in reducing bias since it involves the utilisation of predetermined inclusion criteria, systematic searches and standard data extraction procedures. In comparison to narrative reviews, which are more prone to subjectivity, systematic reviews are carried out through a replicational procedure, which promotes the validity and reliability of the conclusions (Higgins et al., 2019).
The systematic approach used in this paper enabled the systematic review of the current literature on CBT as a non-pharmacological intervention of FAPDs in children and adolescents. The review took a quantitative-oriented synthesis, which places more emphasis on evidence based on randomized controlled trials as the methodology is more powerful in identifying causal relations. Systematic reviews and meta-analyses of RCTs were also incorporated where necessary, as a means of getting aggregated estimates of treatment effects. This focus on RCTs is also due to the fact that they are considered as the gold standard of clinical research especially in clinical interventions (Sibbald and Roland, 1998). The methodological procedure was organized with the help of a number of major steps, such as the research question formulation, the search strategy development, search and selection of the studies, data extraction, and quality evaluation. All the steps were undertaken in a logical and open way to achieve uniformity and minimize chances of bias. Even though the review was not included in an international database like PROSPERO, the review was conducted according to established methodological standards of systematic reviews, which means that it conforms to the best practices in evidence synthesis (Moher et al., 2015).
3.2 PICO Framework
This systematic review was based on the research question developed under the framework of PICO framework, which is one of the most popular tools to frame the clinical research question and direct evidence-based practice (Richardson et al., 1995). Application of the PICO model assisted a clear definition of the population, intervention, comparison and outcomes of interest and thus provided a focused and methodologically sound review process. Interest population The target population was constituted of children and adolescents aged 18 years and below who had been diagnosed with functional abdominal pain disorders. These conditions were characterized in terms of the developed diagnostic criteria, such as the Rome III and Rome IV classifications, frequently employed in clinical practice as well as in research (Hyams et al., 2016). Functional abdominal pain disorders are a set of disorders, including: irritable bowel syndrome, functional dyspepsia, and functional abdominal pain not otherwise specified, and which have in common the presence of chronic or recurrent abdominal pain and the absence of detectable structural abnormalities. The emphasis on pediatric groups is related to the specific clinical and developmental factors of the disorders in children such as the impact of psychological and environmental factors on the manifestation and course of symptoms.

Cognitive behavioral therapy in any structured form was the intervention of interest. CBT is properly proven psychological treatment whose goal is to change the maladaptive behavior and thinking patterns in order to enhance the emotional regulation and strategies of coping with the situation. CBT is postulated to have an effect on the gut-brain axis in the context of FAPDs, where it will mitigate the perception of pain and contribute to overall functioning (Van Tilburg et al., 2015). Types of CBT delivery used in the review were individuals, group based, family-based, and internet-delivered CBT. The fact that several types of delivery are included refers to the dynamism of CBT interventions and their growing accessibility via digital platforms. Standard medical care, placebo interventions, waitlist controls and no treatment were the conditions in the comparator.
The usual standard care was reassurance, dietary counseling, and overall medical management, and placebo interventions, and waitlist controls offered an opportunity of controlling the non-specific effects of treatment. The presence of these comparators made it possible to conduct a full evaluation of the relative efficacy of CBT in various clinical settings. The main outcome of care was the decrease in the abdominal pain that was assessed by the validated tools like Numeric Rating Scale and Visual Analog Scale. Secondary outcomes were health related quality of life, psychological outcomes, that is anxiety and depression and functional outcomes, including school attendance and the level of daily activities. The physical and psychological outcomes are included as FAPDs are multidimensional, and it corresponds with the biopsychosocial model of disease.
3.3 Search Strategy
The search of the literature was carried out in the PubMed database that was chosen because it covers the large range of biomedical and clinical research. PubMed contains journals that have been MEDLINE-indexed and it is generally considered a credible and authoritative source of peer-reviewed literature. The search strategy was aimed at identifying the studies that were relevant and focused on the effectiveness of cognitive behavioral therapy in the pediatric population with the functional abdominal pain disorder. Search terms were created by both a combination of free-text search and controlled vocabulary. The main words were cognitive behavioral therapy, functional abdominal pain, children, and randomized controlled trial. The combination of these terms in the search was done with the help of Boolean operators to improve the sensitivity and specificity of the search. Multiplicity of the synonyms and related terms has also been used to ensure that there would be no possibility of missing the relevant studies especially due to the difference in the terms applied in different studies. A combination of Medical Subject Headings (MeSH) and free-text terms was used to develop the search strategy that is as sensitive and specific as possible.

The search was restricted to studies published in English to ensure feasibility and consistency in data extraction and analysis. Only studies involving human participants were included, and the publication period was limited to the years 2013 to 2023. This time frame was selected to capture contemporary evidence reflecting current clinical practices and advancements in CBT interventions, including the increasing use of digital and internet-based therapies.
In addition to the primary database search, a manual review of reference lists from included studies was conducted to identify any additional relevant articles. This supplementary search strategy is recommended in systematic reviews to minimize the risk of publication bias and ensure comprehensive coverage of the literature (Greenhalgh & Peacock, 2005).
3.4 Inclusion and Exclusion Criteria
The inclusion and exclusion criteria were well defined, and these were developed beforehand and included in the selection of the studies. These criteria were created to make sure that the studies included are relevant and high in quality in their methodology with minimum chances of bias. To be included in the studies the studies had to fulfill the following criteria: the study was a randomized controlled trial or a systematic review based on a randomized controlled trial; the population was a pediatric population aged 18 years or less; the intervention was that of cognitive behavioral therapy; the study also had to report at least one outcome pertinent, e.g., pain reduction, quality of life, or psychological functioning. The involvement of systematic reviews, as well as the primary studies, made it possible to provide a more detailed synthesis of the evidence, especially in instances where meta-analytic information was present. The research studies were excluded when they exclusively studied adult populations, where they used non-randomized research design like an observational study or case reports and where they studied pharmacological intervention without psychological aspect. Studies that could not be translated and interpreted in English were also left out because of the practical considerations. This can bring some language bias but it is a usual practice in systematic reviews which is acceptable in general.
3.5 Study Selection
The selection of the study was done in a systematic and sequential way, which aligned with PRISMA guidelines. A total of 50 records were obtained after the search and screened preliminary with titles and abstracts. At this point, researchers that created an obvious gap to the research question were filtered out, including those that did not include CBT, pediatric groups, and functional abdominal pain disorders. After the first screening, 35 articles were kept to be assessed. All the texts of these studies were subsequently recalled and carefully evaluated against the inclusion and exclusion criteria. This phase included a more strict assessment of the design of the study, population factors, interventions, and results. At this point, studies were filtered out without meeting the predefined criteria or lack of enough data with which to analyze them. Eventually, 10 articles were identified as eligible to take part in the final review. The process of selection was done using a fair and open method and the reasons as to why certain individuals have been excluded at a specific stage have been well documented. The method helps to increase the reproducibility of the review and minimize the selection bias.
3.6 PRISMA Flow Diagram
The study selection process is summarized in a PRISMA flow diagram, which provides a visual representation of the number of studies identified, screened, assessed for eligibility, and included in the review. A total of 50 records were identified through the initial database search.

After screening titles and abstracts, 35 records remained. Of these, 18 full-text articles were assessed for eligibility, and 10 studies were ultimately included in the systematic review. This structured approach ensures transparency and allows readers to assess the comprehensiveness of the search and selection process (Page et al., 2021).
3.7 Data Extraction
A standardized and structured method was employed in collecting the data to ensure consistency and accuracy in the data collection. Each of the included studies was analyzed to extract the relevant information on the following aspects: the study characteristics, methodological details, parameters of the intervention, the outcomes measures, and the key findings. The description of the CBT interventions, their duration, frequency, and method of delivery, in particular, were given special attention as they can determine the effectiveness of the treatment. The data on outcomes were obtained in such a way that it was possible to compare the outcome data among the studies, although the measuring instruments were different. Quantitative data, including effect sizes and statistical significance were noted where possible. The results obtained were summarized into tabular form to make analysis and synthesis easier.
3.8 Quality Assessment
The methodological quality of the articles included in the research were determined with Cochrane Risk of Bias Tool (Higgins et al., 2011), which analyzes possible sources of bias in a variety of areas which are selection bias, performance bias, detection bias, attrition bias and reporting bias. The tool is common in systematic reviews and offers a uniformed framework of determining the quality of the studies. In the evaluation of the features, the majority of studies showed an average methodological quality, and some restrictions were observed. One of the typical problems was the absence of blinding, which is not always easy to implement during the psychological treatment of CBT. Moreover, some of the studies showed a small sample size and this could limit the generalization of the results and decrease the statistical power. Although these limitations exist, the general quality of the evidence was deemed to be enough to make meaningful conclusions.
4. Analysis
4.1 Introduction
This chapter presents a comprehensive analysis of the findings derived from the systematic review of ten selected studies examining the effectiveness of cognitive behavioral therapy (CBT) in children and adolescents diagnosed with functional abdominal pain disorders (FAPDs). The purpose of this analysis is to synthesize the results across studies, identify patterns and inconsistencies, evaluate the strength of evidence, and assess the clinical implications of CBT as a therapeutic intervention.
The analysis is structured around the primary and secondary outcomes identified in the PICO framework, namely pain reduction, quality of life, and psychological outcomes. In addition, this chapter examines variations in intervention delivery, methodological quality, and overall consistency of findings.
4.2 Overview of Included Studies
The ten studies included in this systematic review consisted primarily of randomized controlled trials (RCTs), along with one meta-analysis synthesizing multiple RCTs. Sample sizes ranged from 60 to 200 participants, with study populations consisting exclusively of children and adolescents diagnosed with FAPDs.

Across all studies, CBT was delivered in various formats, including individual sessions, group therapy, family-based interventions, and digital platforms. The heterogeneity of interventions reflects both the adaptability of CBT and the evolving landscape of psychological treatment delivery.
4.3 Analysis of Primary Outcome: Pain Reduction

5. Discussion
5.1 Introduction
The current systematic review sought to determine the efficacy of cognitive behavioral therapy (CBT) in management of functional abdominal pain disorder (FAPDs) in children. The results show a uniformity in evidence that proves that CBT is a viable intervention in terms of minimizing levels of pain, psychological outcomes, and quality of life. In this chapter, critical interpretation of these findings is done against the background of the existing literature including agreements, divergences, and theoretical implications.
5.2 Interpretation of Main Findings
This review evidence shows that CBT has moderate and clinically significant effects on abdominal pain, which is congruent with other meta-analyses. As an example, the results are similar to the results of Korterink et al. (2015), who presented the standardized mean difference of about -0.54 of pain relief in pediatric patients who received psychological therapies.
